ASUHAN KEPERAWATAN KELUARGA PASIEN DENGAN GANGGUAN AMAN NYAMAN PADA KASUS GOUT ARTHRITIS TERHADAP Ny.A DI DESA MULANG MAYA WILAYAH KERJA PUSKESMAS II KOTABUMI KABUPATEN LAMPUNG UTARA TANGGAL 21-25 MARET 2022

Gout Arthritis atau Arthritis piraiadalah suatu peradangan sendi sebagai manifestasi dari akumulasi endepan kristal monosodium urat, yang terkumpul didalam sendi sebagai akibat dari tingginya kadar asam urat didalam darah (Hiperurisemia). Pada tahun 2018, Prevelensi penyakit sendi di lampung berada pada urutan 12 di indonesia yaitu sebesar 7,2%, Data kasus gout arthritis diPuskesmas Kotabumi II selama 3 tahun terakhir terjadi penurunan yaitu tahun 2019sebanyak 1,017 penderita, tahun 2020 sebanyak 803 penderita dan tahun 2021 sebanyak 466 penderita (Rekam Medik Puskesmas Kotabumi II, 2021). Penyakit ini rentan terjadi pada persendian yang dapat mengakibatkan terjadinya nyeri.rumusan masalah pada kasus ini yaitu bagaimana Asuhan Keperawatan dengan Gangguan Aman Nyaman Nyeri Akut pada Kasus Gout Arthritis Terhadap Ny.A.
Tujuan Laporan Tugas Akhir ini adalah mendapatkan gambaran Asuhan Keperawatan secara nyata dengan Gangguan Aman Nyaman pada kasus Gout Arthritis terhadap Ny.A di Wilayah Kerja Puskesmas II Kotabumi Lampung Utara
Dari hasil pengkajian pada kasus ini didapatkan data yaitu klien merasakan nyeri pada persendian dan disertai pembengkakan pada kaki kanan,nyeri dirasa saat melakukan aktifitas berat,nyeri seprti ditusuk-tusuk,TD 120/80 mmhg, RR 21x/m, N 90x/m, S 36,1°C, Urin Acid 7,8 mg/dl Kurang paham tentang masalah yang dialami. Diagnosa keperawatan yang ditegakkan yaitu: Nyeri akut, Gangguan mobilitas fisik,Defisit pengetahuan, Rencana dan Implementasi keperawatan pada Ny.A meliputi 5 TUK, yaitu TUK 1 mampu mengenal masalah, TUK 2 mampu mengambil keputusan, TUK 3 mampu merawat anggota keluarga yang sakit, TUK 4 mampu memelihara kesehatan, TUK 5 mampu memanfaatkan fasilitas pelayanan kesehatan. Evaluas yang dilakukan selama 3 hari perawatan keluarga mampu melakukan TUK 1,2,3,4,5, tetapi masalah nyeri akut teratasi sebagaian dengan skala nyeri 1.
Simpulan dari laporan ini tidak semua yang ada pada tinjauan pustaka terdapat pada kasus Ny. A dengan nyeri akut pada kasus Gout arthritis. Saran dari penulis diharapkan pihak Puskesmas Kotabumi II dapat memberikan asuhan keperawatan dalam menegakkan Diagnosa Keperawatan berdasarkan scoring dan juga dapat menerapkan terapi komplementer yang merupakan salah satu terapi dalam keperawatan

ABSTRACT

Gout Arthritis atau Arthritis Gout is an inflammation of the joints as a manifestation of the accumulation of monosodium urate crystal deposits, whichcollect in the joints as a result of hight levels of urin acid in the blood (hyperuricemia). In 2018,the prevalence of joint disease in lampung was at number 12 in indonesia, which was 7,2%. Data on gout arthritis cases at the kotabumi II Health Center for the last 3 years descreasd, namely in 2019 as many as 1,017 patients, in 2020 as many as 803 patients, 2021 as many as 446 patients,(Medical Records of Kotabumi II Health Center,2021). This disease is prone to accur in the joints which can couse pain. The formulation of the problem in this case is how nursing care with safe and comfortable disorders of acute pain in cases of Gout Arthritis Against Ny.A
The purpose of this final report is to get a real picture of Nursing Care with Safe and Comfortable Disorders in the case of Gout Arthritis against Mrs.A in the Work Area of Puskesmas II Kotabumi , North Lampung
Form the results of the study in this cae, it was found that the client felt pain in the joints and accompanied by swelling in the right leg, the pain was felt when soing strenousactivities such as being stabbed. TD 120/80 mmHg, RR 21x/m. N 90x/m, S 36,1°C, Urin Acid 7,8 mg/dl lack pof understanding about the problems experience. Nursing of diagnosesm that are enforced are: Acute pain, Physical mobility impairment, knowledge degicit. Nursing plan and implementation for Mrs.A includes 5 TUK, namely TUK 1 is able to recognize problem, TUK2 is able to make decisions, TUK 3 is able to care for sick family members, TUK 4 is able to maintain health, TUK 5 is able to ulilize health service facilities. The evolution carried our for 3days of family care was acle to do TUK 1,2,3,4,5 but the acute pain problem was partially resolved with a pain scale 0f 1.
The conclusion from this report is that not everything in the literature review is found in the case of Mrs.A with acute pain in a case of Gout Arhtritis. Suggestions from the aouthor it is hoped that the Kotabumi II Health Center can provide nursing care inenforcing nuersing diagnoses based on scoring and can also provide nursingcare.
