ASUHAN KEPERAWATAN PERIOPERATIF PADA PASIEN ULKUS DIABETIKUM DENGAN TINDAKAN AMPUTASI DIGITI II PEDIS DEXTRA DI RSUD JENDRAL AHMAD YANI METRO TAHUN 2022

Diabetes mellitus merupakan penyakit metabolik yang disebabkan oleh difisiensi insulin, yang menyebabkan komplikasi ulkus pada kaki. Tatalaksana pada ulkus diabetikum salah satunya amputasi. Tujuan laporan tugas akhir ini melakukan asuhan keperawatan perioperatif mulai dari kondisi pre operasi, intra operasi, dan post operasi, pada pasien Ulkus Diabetikum dengan tindakan Amputasi Digiti II Pedis Dextra di RSUD Jendral Ahmad Yani Metro Tahun 2022.
Metode asuhan keperawatan perioperatif menggunakan pendekatan proses keperawatan, sample pada pasien yang dilakukan tindakan amputasi digiti dengan diagnosa ulkus pedis, Berdasarkan hasil pengkajian dirumuskan beberapa diagnosa keperawatan sebagai berikut : pre operasi ; ansietas, intra operasi ; resiko perdarahan, dan post operasi ; resiko perdarahan, nyeri akut, gangguan integritas kulit/ jaringan dan defisit perawatan diri, Rencana keperawatan berdasarkan diagnosa yang telah dirumuskan, implementasi tindakan dikerjakan sesuai intervensi keperawatan yang ditetapkan secara mandiri maupun kolaborasi, sehingga tujuan rencana Tindakan tercapai, Evaluasi dari setiap diagnosa keperawatan didapatkan sebagai berikut, tahap pre operasi ansietas teratasi, gangguang integritas kulit/ jaringan teratasi dengan dilakukannya tindakan pembedahan, tahap intra operasi resiko perdarahan tidak terjadi. Tahap post operasi masalah resiko perdarahan tidak terjadi, nyeri akut teratasi, gangguan integritas kulit/ jaringan dan defisit perawatan diri teratasi. Saran, Diharapkan dapat melakukan prosedur asuhan keperawatan sesuai dengan standar yang berlaku sesuai dengan tahapan pengkajian, perumusan diagnosa keperawatan, pembuatan intervensi, pelaksanaan implementasi dan evaluasi baik saat pre operasi, intra operasi, maupun post operasi

Perioperative Nursing Care for Diabetic Ulcer Patients with Digiti II Dextra Pedis Amputation in RSUD Jendral Ahmad Yani Metro 2022

ABSTRACT
Diabetes mellitus is a metabolic disease caused by insulin deficiency, which causes complications of foot ulcers. One of the treatment for diabetic ulcers is amputation. The purpose of this final report is to carry out perioperative nursing care starting from preoperative, intraoperative, and postoperative conditions, in Diabetic Ulcer patients with Digiti II Pedis Dextra Amputation at General Ahmad Yani Metro Hospital in 2022.
The perioperative nursing care method uses a nursing process approach, a sample of patients who underwent digital amputation with a diagnosis of pedis ulcer was carried out. Based on the results of the study, several nursing diagnoses were formulated as follows: preoperative; anxiety, intraoperative; risk of bleeding, and post surgery; risk of bleeding, acute pain, impaired skin/tissue integrity and self-care deficits, nursing plans are based on formulated diagnoses, implementation of actions is carried out according to nursing interventions that are determined independently or collaboratively, so that the objectives of the action plan are achieved, evaluation of each nursing diagnosis is obtained as Following this, the pre-operative stage of anxiety is resolved, skin/tissue integrity disorders are resolved by surgery, the intra-operative stage of bleeding risk does not occur. In the post-operative stage, the risk of bleeding does not occur, acute pain is resolved, impaired skin/tissue integrity and self-care deficits are resolved. Suggestion, It is hoped that nursing care procedures can be carried out in accordance with applicable standards in accordance with the stages of assessment, formulation of nursing diagnoses, making interventions, implementing and evaluating both preoperative, intraoperative, and postoperative.
